Whisk the cocoa powder, sugar, and flour in a saucepan until no lumps remain.

Slowly whisk in the milk until the mixture is completely smooth.

Cook over medium heat on the stovetop. Stir constantly until it thickens like gravy, about 8-10 minutes.

Take the pan off the heat. Stir in the butter and vanilla extract.
